Important information about Enbrel

The active ingredient in Enbrel is etanercept, which is a TNF inhibitor. TNF is a protein made by your immune system that helps drive inflammation. In conditions like rheumatoid arthritis and psoriasis, TNF levels can be higher than normal and contribute to ongoing inflammation. Etanercept’s mechanism of action involves acting like a “decoy” TNF receptor, binding TNF, and blocking it from attaching to cell receptors. This reduces TNF activity, which calms inflammation in the body. Because of its immunosuppressive activity, Enbrel is associated with an increased risk of certain side effects that you should discuss with your prescriber.

While using Enbrel, your prescriber may have you continue your other medications (such as methotrexate or NSAIDs) to help manage your condition.

However, the following is a synopsis of the general guidelines for the use of Enbrel:

  • Enbrel is given as a subcutaneous injection (an injection under the skin). Some people get Enbrel from a healthcare provider, while others may be trained to inject it at home (or have a caregiver inject it). Do not try to inject Enbrel until a healthcare provider has shown you the correct technique.
  • Enbrel is available in different dosage forms, such as a prefilled syringe, SureClick autoinjector, single-dose vial, multiple-dose vial, and Enbrel Mini cartridge used with the AutoTouch reusable autoinjector. Your healthcare provider will choose the right formulation for you.
  • Use Enbrel exactly as prescribed and do not use it more often than directed. Your dosing schedule depends on your medical condition and, for children, body weight.
  • If you miss a dose, inject it as soon as you remember. Then take your next dose at your regularly scheduled time. If you are not sure when to take your next dose, call your healthcare provider or pharmacist.

Some mild or common side effects of Enbrel seen in clinical trials as compared to placebo include:

  • Injection site reactions are common. These may include redness, itching, pain, swelling, bleeding, or bruising where the injection was given.
  • Upper respiratory infections (such as sinus infections) are also reported.

Some individuals may experience serious side effects while using Enbrel. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:

  • Serious infections: Enbrel can lower your immune system’s ability to fight infection. Serious infections, including tuberculosis (TB) and infections caused by bacteria, viruses, or fungi (including fungal infections), have occurred, and some people have died. Call your healthcare provider right away if you develop signs of infection such as fever, chills, cough, shortness of breath, weight loss, diarrhea or stomach pain, painful urination, or extreme tiredness.
  • Cancer risk: Lymphoma and other cancers have been reported in people using TNF blockers, including Enbrel, and unusual cancers have been reported in children and adolescents.
  • Hepatitis B reactivation: If you had hepatitis B in the past, the virus may become active again during treatment.
  • Nervous system problems: Rare nervous system reactions (including multiple sclerosis-like events, seizures, or eye nerve inflammation). Seek medical advice right away for symptoms such as numbness/tingling, vision changes, weakness, or dizziness.
  • Blood problems: Low blood counts (blood cells that help fight infection or help stop bleeding) have been seen with TNF blockers. Get help for symptoms like unusual bruising/bleeding, fever, or looking pale.
  • Heart failure: New or worsening congestive heart failure can occur. Get medical attention for worsening shortness of breath or swelling in the legs/feet.
  • Allergic reactions: Serious allergic reactions can happen. Get emergency help if you develop a severe rash, hives, swelling of the face, or trouble breathing.
  • Autoimmune reactions: Lupus-like syndrome and autoimmune hepatitis have been reported; call your healthcare provider if you develop concerning symptoms such as rash that worsens in the sun, yellowing of the skin/eyes, vomiting, or right-sided stomach pain.

These are not all of the possible side effects of Enbrel. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Before starting Enbrel, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:

  • Any infection now or one that keeps coming back, diabetes, HIV, or a weak immune system (these can increase your risk of serious infections).
  • A history of TB or close contact with someone who has TB, or if you have lived in or traveled to areas where TB or certain fungal infections are more common. Your healthcare provider should test you for TB before starting Enbrel and monitor you during treatment.
  • Been around someone with varicella zoster (chicken pox).
  • Hepatitis B now or in the past (reactivation can happen).
  • Heart failure, nervous system problems (including multiple sclerosis or Guillain-Barré syndrome), or blood cell count abnormalities.
  • Upcoming surgery.
  • Have received or are planning to receive live vaccines. Discuss your immunization history with your prescriber before starting Enbrel.
  • Pregnancy or breastfeeding. If Enbrel was used during pregnancy, talk with the healthcare provider about live vaccine timing for the infant.

Enbrel should not be used in patients with sepsis (an infection that has spread through the body).

Some people may notice symptom improvement over time with ongoing treatment, but results can vary. Your healthcare provider can tell you what to expect based on your diagnosis and treatment plan.

When Enbrel is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects.   • Orencia (abatacept) or Kineret (anakinra). Using Enbrel with these medicines may increase the chance of serious infections.
  • Cyclophosphamide (Cytoxan). Using Enbrel with cyclophosphamide may increase the chance of certain cancers.
  • Anti-diabetic medicines. If you have diabetes, your healthcare provider may need to adjust your diabetes medicines while you are taking Enbrel.
  • Vaccines: People taking Enbrel should avoid live vaccines. Ask your healthcare provider about vaccination needs before and during treatment.

Enbrel comes in multiple dosage forms and strengths, including:

Injection: 25 mg/0.5 mL and 50 mg/mL solution in a single-dose prefilled syringe

Injection: 50 mg/mL solution in a single-dose prefilled SureClick autoinjector

Injection: 25 mg/0.5 mL solution in a single-dose vial

For injection: 25 mg lyophilized powder in a multiple-dose vial for reconstitution

Injection: 50 mg/mL solution in Enbrel Mini single-dose prefilled cartridge for use with the AutoTouch reusable autoinjector

Store Enbrel in the refrigerator at 36°F to 46°F (2°C to 8°C) and keep it in the original carton to protect from light. Do not freeze or shake.

Some Enbrel products can be stored at room temperature (68°F to 77°F / 20°C to 25°C) for a limited time (for example, up to 30 days for certain single-dose products). Once Enbrel has reached room temperature, do not put it back in the refrigerator. Do not store Enbrel in extreme heat or cold (such as in a vehicle). Keep all medicines out of reach of children.
