What is Nucala?
Nucala (mepolizumab) is a biologic medicine (a monoclonal antibody) that is FDA-approved as an add-on treatment for certain conditions linked to high levels of eosinophils, a type of white blood cell that can drive inflammation. This includes the add-on maintenance treatment of severe eosinophilic asthma in adults and adolescents 6 years of age and older, chronic rhinosinusitis with nasal polyps (CRSwNP) in adults, and chronic obstructive pulmonary disease (COPD) in adults with an eosinophilic phenotype who are not well controlled and are at risk of flare-ups (exacerbations). It is also approved to treat eosinophilic granulomatosis with polyangiitis (EGPA) in adults and hypereosinophilic syndrome (HES) in people 12 years of age and older who have had the condition for 6 months or more without an identifiable non-hematologic secondary cause.
Nucala is not a rescue medicine. It is not used to treat sudden breathing problems (acute bronchospasm) or status asthmaticus (asthma attack). Keep using your quick-relief inhaler and other medicines as directed by your healthcare provider.

Important information about Nucala
The active ingredient in Nucala is mepolizumab, which is an interleukin-5 (IL-5) antagonist monoclonal antibody. IL-5 is a signal in the body that helps eosinophils grow and survive. By blocking IL-5, Nucala lowers blood eosinophil levels, which may reduce the inflammation that contributes to severe eosinophilic asthma, CRSwNP, COPD, EGPA (a type of vasculitis), and HES. These effects of Nucala can help improve asthma control and reduce exacerbations for the conditions it is approved to treat.
Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Nucala. However, the following is a synopsis of the general guidelines for the use of Nucala:
- Nucala is given as a subcutaneous injection (an injection under the skin) every 4 weeks. A healthcare professional may give the Nucala injection, or in some cases, a patient or caregiver may give it at home using a single-dose prefilled syringe or single-dose autoinjector after training. Common injection sites include the thigh or stomach (abdomen). A caregiver may also inject into the upper arm.
- The dose depends on what condition you are being treated for and your age.
- Do not stop your oral corticosteroids, systemic corticosteroids, or inhaled corticosteroids suddenly unless your healthcare provider tells you to. Your corticosteroid dose may need to be lowered slowly.
- If you miss a dose, take it as soon as possible and then resume your regular schedule. If you do not realize you missed a dose until it is time for your next dose, take the next dose as planned. If you are not sure when to inject, call your healthcare provider.
Some mild or most common side effects of Nucala seen in clinical trials as compared to placebo include:
- Headache
- Injection site reaction (pain, redness, swelling, itching, or burning)
- Back pain
- Tiredness
Some individuals may experience serious side effects while using Nucala. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:
- Serious allergic reaction (hypersensitivity reactions), including anaphylaxis and angioedema. These reactions can happen after the injection and may occur hours or days later. Get emergency help right away if you have breathing problems, hives, rash, fainting, dizziness, low blood pressure, or swelling of your face, mouth, or tongue.
- Herpes zoster infections (shingles) have happened in people receiving Nucala.
These are not all of the possible side effects of Nucala. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the U.S. Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.
Before starting Nucala, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:
- A history of allergic reaction to mepolizumab or any ingredient in the formulation, since serious hypersensitivity reactions can occur.
- Acute asthma symptoms or sudden breathing problems from asthma or COPD. Nucala is not meant for quick relief and should not be used for acute bronchospasm or status asthmaticus.
- A parasitic (helminth) infection. People with pre-existing helminth infections should be treated before starting Nucala. If a helminth infection happens during treatment and does not respond to therapy, your provider may stop Nucala until the infection is resolved.
- Current use of nasal corticosteroids or oral/systemic corticosteroids. Do not stop steroids suddenly. Your healthcare professional may adjust your steroid dose gradually.
- Vaccination needs. Herpes zoster infections have occurred with Nucala, and vaccination may be considered if medically appropriate.
Nucala is contraindicated in people with a history of hypersensitivity to mepolizumab or any excipients (inactive ingredients) in the formulation.
Nucala is an add-on maintenance treatment meant for long-term control rather than quick relief. Many people notice fewer flare-ups (exacerbations) over time, but the response can vary based on your disease, baseline eosinophil count, and overall phenotype. Keep taking it on schedule and follow up with your healthcare provider to track symptom control and flare frequency.
When Nucala is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions.
Nucala is available in these FDA-approved dosage forms and strengths:
- For injection: 100 mg lyophilized powder in a single-dose vial for reconstitution (given by a healthcare provider)
- Injection: 100 mg/mL single-dose prefilled autoinjector or single-dose prefilled syringe
- Injection: 40 mg/0.4 mL single-dose prefilled syringe
Store prefilled autoinjectors and prefilled syringes in the refrigerator and keep them in the original carton to protect from light. Do not freeze or shake, and keep away from heat. If needed, an unopened carton may be stored outside the refrigerator at up to room temperature for up to 7 days. Throw it away if it has been out for more than 7 days. Once removed from the carton, the prefilled autoinjector or syringe must be used within 8 hours or safely thrown away.
For single-dose Nucala vials, do not freeze, and keep in the original carton to protect from light.
Nucala is manufactured by GlaxoSmithKline (GSK).
No, there is no generic version of Nucala available.
Do not stop Nucala or any of your other medicines unless your healthcare provider tells you to. If you are using oral corticosteroids, systemic corticosteroids, or inhaled corticosteroids, do not stop them suddenly when starting or stopping Nucala. Your healthcare provider may lower your corticosteroid dose gradually if appropriate.
