The active ingredient in Tremfya, guselkumab, is a monoclonal antibody. More specifically, it is an interleukin-23 (IL-23) inhibitor. The mechanism of action involves blocking IL-23 receptors that typically trigger your immune system. Tremfya reduces inflammation (swelling) in the body to treat the symptoms of psoriatic arthritis and plaque psoriasis by inhibiting IL-23.

However, the following is a synopsis of the general guidelines for the use of Tremfya:

  • Tremfya is administered as a subcutaneous injection to patients with psoriatic arthritis or plaque psoriasis.
  • In a medical facility, a healthcare provider may administer the first (induction) doses to patients with ulcerative colitis via an intravenous infusion, which is administered through a vein in the arm. Patients will then receive subcutaneous injections that can be self-administered or given by a caregiver following the completion of the induction dosages.
  • Tremfya is injected beneath the skin as part of the procedure. You will receive instructions on how to administer shots at home. If you do not know how to administer the injection or dispose of used syringes and needles appropriately, do not give yourself this medication.
  • Depending on the condition being treated and the dosage, the first two injections are typically administered four weeks apart (induction treatment) and then every four to eight weeks for maintenance treatment. The frequency and duration of this treatment will be decided by your physician.
  • Tremfya should not be injected into an active psoriasis lesion. Steer clear of injecting into red, raw, bruised, thick, or scaly skin.
  • Every prefilled syringe or One-Press injector is only intended for one usage. Even if there is still medication inside, discard it after one use.

The most common side effects of Tremfya seen in clinical trials include:

  • Headaches.
  • Joint pain (arthralgia).
  • Increased risk of infections, including upper respiratory tract infection, bronchitis, or a common cold, fungal infection, and herpes simplex infection.
  • Stomach flu (gastroenteritis).
  • Diarrhea.
  • Stomach pain.
  • Weight loss.
  • Injection site reactions.

Before starting Tremfya, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:

  • An allergy to polysorbate 80, guselkumab, or any of the other inactive ingredients in Tremfya, you should avoid using this medication.
  • Anyone in your family has tuberculosis, or you have had the disease yourself. Additionally, if you have recently traveled, let your doctor know. In some regions of the world, tuberculosis and some fungal infections are more prevalent, and you might have been exposed while traveling.
  • An ongoing or persistent infection.

  • Liver disease. Your liver enzymes and bilirubin levels will be checked at baseline, during the first 16 weeks of treatment for Crohn's disease or severely active ulcerative colitis, and then at regular intervals after that, by standard patient care. Your treatment will be stopped until drug-induced liver injury is ruled out if it is suspected.

Those who have previously experienced a serious hypersensitivity reaction to any of the excipients or to guselkumab.

The full effects of this medicine may not be felt for several months. After roughly four months of taking Tremfya for plaque psoriasis, you may notice a clearer complexion. And after roughly six months of using this drug for psoriatic arthritis, you might notice a reduction in your symptoms, such as joint pain, stiffness, and edema.

When Tremfya is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Tremfya is available in the following dosage forms and strengths: 100 mg/mL and 200 mg/2 mL for subcutaneous injection and a 200 mg/20 mL (10 mg/mL) single dose vial for intravenous infusion.

The typical dose for active psoriatic arthritis and moderate to severe plaque psoriasis is usually the same. One single-dose vial injection will be given to you at week 0 and again at week 4. After that, you'll get an injection every eight weeks.

Week 0 (first day of treatment): Inject 100 mg under the skin once.

Week 4: Inject 100 mg under the skin once.

Week 12 and every 8 weeks afterward: Inject 100 mg under the skin once.

Keep this medication refrigerated. Avoid freezing and shield from light.

Before injecting your dose, remove the medication from the refrigerator and let it come to room temperature for half an hour. Wait until you are ready to administer the injection before removing the needle cap.
