What is Xolair?
What is Xolair?
Xolair (omalizumab) is an IgE monoclonal antibody that is used to treat allergic severe persistent asthma, chronic spontaneous urticaria (CSU), and nasal polyps when first-line treatments do not manage the symptoms. This drug is also used to reduce the likelihood of IgE-mediated food allergy reactions following accidental exposure to triggers. Xolair should not be used to treat acute asthma or emergency allergic reactions including anaphylaxis.
Xolair can be used by adults, adolescents, and children 6 years of age and older, depending on the condition being treated. It is administered as an injection beneath the skin, usually every 2 or 4 weeks.

Important information about Xolair
The active ingredient in Xolair is omalizumab. Omalizumab is a biological medication that works by specifically targeting immunoglobulin E (IgE), an immune system protein. This protein is often involved in boosting your immune system's allergic response to anything, such as certain food allergens. Xolair treatment prevents IgE from binding to its receptors and generating histamine and other substances that cause allergic asthma symptoms, chronic rhinosinusitis with nasal polyps (CRSwNP), hives, and food allergies. This also decreases inflammation of the airways and makes it easier to breathe.
Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Xolair. However, the following is a synopsis of the general guidelines for the use of Xolair:
- Xolair is administered as a subcutaneous (under the skin) injection, usually in the upper arms, stomach, or thighs. When you first begin treatment, your healthcare professional will provide the injection. You will be closely monitored for a short time after having the injection to ensure that you do not develop an allergic reaction to the drug.
- Your healthcare practitioner may decide that you or a carer can administer the injections, and you will be trained on how to properly prepare and inject this medication. Do not attempt to inject Xolair unless you have been shown how to properly use the prefilled syringes or autoinjectors.
- The prefilled syringe can be self-injected by youngsters aged 12 and up, with adult supervision. For children aged 6 to 11 years of age, a caregiver should inject the prefilled syringe.
- Xolair is commonly administered every 2 to 4 weeks, depending on the condition being treated.
- If you also use a corticosteroid, do not quit abruptly because you may experience serious side effects. Consult your doctor if any of your asthma drugs do not appear to be working as well in treating or avoiding attacks.
- While using this medication, you may require regular medical testing, such as allergy tests and lung function tests. Your stools may also need to be examined for parasites, particularly if you travel.
- Your condition may not improve immediately. For optimal effects, continue taking the medication as prescribed. Consult your doctor if your symptoms do not improve after a few weeks of medication.
The most common side effects of Xolair seen in clinical trials include:
- Joint pain or body pain.
- Injection site reactions.
- Dizziness.
- Fatigue.
- Headaches.
- Common flu-like symptoms
- Upper respiratory tract infection.
- Sinus infection.
- Upper abdominal pain.
Some individuals may experience serious side effects while using Xolair. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:
- Serious allergic reactions or angioedema: hives, wheezing, difficulty breathing, and swelling of the face, lips, tongue, or throat.
- Inflammation of blood vessels: rash, difficulty breathing, tingling or numbness in arms or legs, and chest pain.
- Heart and blood circulation issues: chest pain, heart attacks, blood clots, and strokes.
- Increased risk of parasitic infections.
These are not all of the possible side effects of Xolair. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088. You may also report side effects to Genentech at (888) 835-2555 or Novartis Pharmaceuticals Corporation at (888) 669-6682.
Before starting Xolair, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:
- Some persons who have had this injection have experienced a severe, life-threatening allergic reaction either immediately following the injection or hours later. Allergic reactions may occur even after using this injection regularly for a year or more.
- If you experience any symptoms of an allergic response to Xolair, get emergency medical attention. These include hives, rash, anxiety or worry, flushing (warmth, redness, or tingling sensation), feeling like you could pass out, chest tightness, wheezing, shortness of breath, difficulty breathing, fast or slow heartbeats, swelling of your cheeks, lips, tongue, or neck.
- Xolair should not be used as an emergency treatment for allergic responses, such as anaphylaxis. It is only used in food allergies to lessen the likelihood of allergic responses.
- If you also use an oral steroid medicine, do not quit abruptly as you may get unpleasant withdrawal symptoms. Consult your doctor if any of your asthma drugs do not appear to be working as well in treating or avoiding attacks.
To ensure that Xolair is safe for you, inform your doctor if you have:
- Any indications of illness (fever, swollen glands, general malaise).
- Any other allergies (food, pollen, etc.)
- Parasite-caused infection.
- Past or present cancer.
- History of a heart attack or stroke.
- If you're getting allergy injections.
- A serious allergic reaction (anaphylaxis).
- If you have a latex allergy, the prefilled syringe's needle cap contains a natural rubber latex.
- This injection may raise your risk of developing malignancies (cancer) of the breast, skin, prostate, or salivary gland. Consult your doctor about your risk.
- If you reside in or go to locations where parasites (worms) are prevalent, you may be at a higher risk of contracting them while taking this medication. Discuss with your doctor what to watch for and how to treat this illness.
The only contraindication of Xolair is if you have a severe allergic reaction or hypersensitivity to Xolair, then do not take this medication.
The time it takes for Xolair (omalizumab) to work will vary and depend on the medical condition being treated. According to studies, some persons experienced improvements in their symptoms within 3 months for the treatment of persistent hives, 4 months for the treatment of allergic asthma, 5 months for the avoidance of severe allergic reactions to food, and 6 months for the treatment of nasal polyps. Keep in mind that everyone reacts to Xolair differently. If you are concerned about the effectiveness of Xolair for your medical condition, consult your allergist or healthcare provider.
When Xolair is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions.
This includes any of the following: allergy shots (a series of injections given regularly to prevent the body from developing allergic reactions to specific substances) or immune-suppressing drugs. Your doctor may need to adjust the dosages of your drugs or closely monitor you for negative effects.
Your allergist will determine and change your Xolair dose based on a variety of factors such as your baseline IgE level, body weight (kg), and how you respond to the treatment.
Xolair is injected under the skin (subcutaneous or SC) in the front and middle of your thigh, the stomach area (2 inches from your belly button), or the back of your upper arm (if administered by a carer).
