The active ingredient in Adempas is riociguat.

Riociguat is a soluble guanylate cyclase stimulator. The mechanism of action is that it increases the activity of an enzyme that helps the blood vessels in the lungs relax. This raises levels of cyclic guanosine monophosphate, improves circulation, and lowers pulmonary artery pressure. Many patients notice better breathing and walking ability once treatment is established.

Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Adempas. However, the following is a synopsis of the general guidelines for the use of Adempas:

  • Adempas is taken by mouth three times a day.
  • Space doses about six to eight hours apart.
  • You may take it with or without food. Tablets may be crushed and mixed with soft food like applesauce if needed.
  • Treatment usually begins with a low starting dose. Your healthcare provider may adjust your dose based on your blood pressure and symptoms.
  • If you miss a dose, take your next dose at the usual time. Do not double up on doses.
  • Smoking lowers the level of riociguat in the body. If you smoke, your doctor may adjust your dose.
  • Only prescribers and pharmacies enrolled in the Adempas REMS Programs can provide this medication due to the risk of birth defects.

Some mild or common side effects of Adempas seen in clinical trials as compared to placebo include:

  • Headache.
  • Dizziness or lightheadedness.

Boxed Warning

Adempas carries a boxed warning for embryo-fetal toxicity. It can cause serious birth defects or pregnancy loss. Females of reproductive potential must enroll in the Adempas REMS Programs, complete regular pregnancy tests, and use reliable contraception.

  • Pregnancy.
  • Use with nitrates or nitric oxide donors, including nitroglycerin or amyl nitrite.
  • Use with PDE-5 inhibitors such as sildenafil, tadalafil, or vardenafil.
  • Use with other soluble guanylate cyclase stimulators.
  • Known hypersensitivity to riociguat or any tablet ingredients.

Some patients see symptom improvement within several weeks. In clinical trials, improvements in exercise capacity and walk distance were generally seen within 12 to 16 weeks. The timeline may vary depending on your condition and WHO functional class.

When Adempas is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:

  • Nitrates and nitric oxide donors - risk of severe low blood pressure.
  • PDE-5 inhibitors such as sildenafil, tadalafil, or vardenafil may cause dangerous drops in blood pressure.
  • Strong CYP or P-gp inhibitors such as ketoconazole or itraconazole may increase riociguat levels.
  • Strong CYP inducers such as St. John’s wort may reduce the effect of riociguat.
  • Theophylline -may lessen the effect of riociguat.
  • Antacids - may lower riociguat absorption if taken within one hour of dosing.

1 mg taken orally three times a day as the starting dose, and 2.5 mg three times a day is the maximum dosage. An initial dose of 0.5 mg taken orally three times a day should be considered for those who are unable to handle the hypotensive effect.

The dosage may be increased by 0.5 mg taken orally three times a day if the patient's systolic blood pressure is still higher than 95 mmHg and there are no indications of hypotension. The frequency of dose increases should not exceed every two weeks.

Store at room temperature and keep out of reach of children.

No generic version of riociguat is currently available in the United States or Canada.

Do not stop taking Adempas without speaking with your healthcare provider. If you stop taking it for several days, your healthcare provider may restart your plan at a lower dose.

Talk to your doctor if you are already pregnant or if you have plans to become pregnant. Adempas can cause severe birth defects and should not be used in pregnancy. Females can only receive Adempas through a restricted program called the Adempas Risk Evaluation and Mitigation Strategies (REMS) program.

Talk to your doctor if you are breastfeeding or planning to breastfeed. It is unknown if riociguat passes into breast milk, and your doctor will guide you on the safest approach.
