Important information about Daliresp

The active ingredient in Daliresp is roflumilast. Roflumilast belongs to a class of medications known as selective phosphodiesterase 4 inhibitors (PDE4 inhibitors). Although it is unknown how exactly Daliresp benefits COPD patients, it is thought that these medicines, by stopping the PDE4 enzyme, increase cyclic AMP in the lungs. This change is associated with an improvement in FEV1, a key indicator of lung function. As a result, patients with a history of exacerbations and severe COPD with chronic bronchitis will see an improvement in their symptoms. It is important to note that Daliresp is not a bronchodilator. Therefore, it cannot be used for bronchospasm attacks, where someone experiences a sudden onset of wheezing or coughing. In these cases, a short-acting medication with a fast onset is preferred.

Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide, before using Daliresp. However, the following is a synopsis of the general guidelines for the use of Daliresp:

  • Daliresp should be used exactly as prescribed.
  • Daliresp is usually taken daily with or without food. You will typically be started on an initial dose of 250 mcg daily for 4 weeks before your healthcare prescriber considers you for a dose increase.
  • If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.

The most common side effects of Daliresp seen in clinical trials as compared to placebo include:

  • Diarrhea
  • Weight loss
  • Loss of appetite
  • Nausea
  • Headache
  • Back pain
  • Muscle spasms
  • Flu-like symptoms
  • Insomnia or trouble sleeping.
  • Dizziness

Some individuals may experience serious side effects while using Daliresp. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:

  • Signs of mental health problems such as thoughts of suicide, trouble sleeping, new or worsening anxiety, new or worsening depression, or other unusual changes in mood or behavior. Patients and caregivers should report such changes immediately to their healthcare provider.
  • Extensive weight loss. Daliresp can cause decreased appetite, so body weight should be monitored throughout treatment. Any large losses should be followed up on by your healthcare provider.

These are not all of the possible adverse reactions of Daliresp. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.

Before starting Daliresp, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including whether you have:

  • A history of mental health problems, including depression and suicidal thoughts or behaviors
  • Liver problems
  • Are pregnant or plan to become pregnant
  • Are breastfeeding or plan to breastfeed

Do not use Daliresp if you have moderate to severe liver problems.

It may take several weeks of regular use to see the full effects of roflumilast or Daliresp.

When Daliresp is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:

  • Rifampicin
  • Phenobarbital
  • Carbamazepine
  • Phenytoin
  • Erythromycin
  • Ketoconazole
