Important information about Xigduo XR
The active ingredient in Xigduo XR is dapagliflozin and metformin hydrochloride. Dapagliflozin belongs to the sodium-glucose cotransporter 2 or SGLT2 inhibitor drug class. It works in the kidneys by helping remove excess glucose from the body through the urine. Metformin is a biguanide that reduces the amount of glucose made by the liver, decreases glucose absorption from food, and improves the body’s response to insulin. Together, these medicines help lower blood sugar levels without directly increasing insulin production.
Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Xigduo XR. However, the following is a synopsis of the general guidelines for the use of Xigduo XR:
- Take Xigduo XR once daily with food to reduce stomach pain and improve tolerability.
- Swallow the extended-release tablets whole. Do not crush, cut, or chew.
- Take the medication at the same time each day.
- If you miss a dose, take it as soon as you remember unless it is close to your next dose. Do not take two doses at once.
- Continue your recommended diet, glucose tests, and exercise plan.
- Your healthcare provider may adjust the dose of metformin mg based on your blood glucose and kidney function.
Some mild or common side effects of Xigduo XR seen in clinical trials as compared to placebo include:
- Yeast infection or genital infection.
- Urinary tract infection.
- Stomach pain, diarrhea, or nausea.
- Headache.
- Runny nose or sore throat.
- Low blood sugar when used with insulin or other antidiabetic drugs.
- Increased urination.
- Vitamin B12 deficiency with long-term use.
- Weight loss.
Some individuals may experience serious side effects while using Xigduo XR. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:
- Lactic acidosis, which may cause muscle pain, trouble breathing, unusual sleepiness, or lactic acid buildup.
- Diabetic ketoacidosis (increased ketones in your blood or urine), even if blood sugar levels are not very high.
- Severe urinary tract infection, urosepsis, or pyelonephritis.
- Fournier’s gangrene or necrotizing fasciitis of the genital area.
- Severe dehydration, hypotension, or low blood pressure with lightheaded feeling.
- Kidney problems or a sudden decrease in urine.
- Pancreatitis.
- Allergic reaction with hives, swelling, or trouble breathing.
- High blood sugar or hyperglycemia that does not improve.
- Signs of metabolic acidosis.
These are not all of the possible side effects of Xigduo XR. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.
Before starting Xigduo XR, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including:
- Kidney disease or reduced kidney function.
- Liver disease.
- Heart failure, heart disease, or heart attack history.
- Low blood pressure or dehydration.
- A history of pancreatitis (inflammation of the pancreas).
- Frequent urinary tract infection or yeast infection.
- Low vitamin B12 levels.
- High alcohol intake.
- Planned surgery or X-ray procedures using contrast dye.
- Risk factors for lactic acidosis, such as severe infection or reduced oxygen supply.
Boxed Warning
- Metformin can cause lactic acidosis, a rare but life-threatening condition. The risk is higher in people with kidney problems, liver disease, severe infection, dehydration, or those who drink alcohol excessively. Symptoms may include weakness, stomach pain, muscle pain, trouble breathing, and feeling cold. Stop taking Xigduo XR and seek medical attention right away if these occur.
Xigduo XR should not be used in patients with:
- Severe kidney disease or end-stage renal disease.
- Metabolic acidosis, including diabetic ketoacidosis.
- A known allergic reaction to dapagliflozin, metformin, or any inactive ingredients.
Xigduo XR begins lowering blood glucose within the first few days, but full improvement in glycemic control and A1C may take several weeks to a few months. Your healthcare provider will monitor your blood glucose and adjust treatment as needed.
When Xigduo XR is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:
- Diuretics such as hydrochlorothiazide, which may increase the risk of dehydration and hypotension.
- Cimetidine, which can raise metformin levels.
- Topiramate and other carbonic anhydrase inhibitors, which may increase the risk of metabolic acidosis.
- Insulin or sulfonylureas, which may increase the risk of hypoglycemia.
- Alcohol, which raises the risk of lactic acidosis.
- Contrast dye used for X-ray procedures, which may affect kidney function.
- Supplements or herbal products that affect blood sugar.
Xigduo XR is available as extended-release tablets in multiple dosage forms, including:
- Dapagliflozin 5 mg/metformin hydrochloride 500 mg.
- Dapagliflozin 5 mg/metformin hydrochloride 1000 mg.
- Dapagliflozin 10 mg/metformin hydrochloride 500 mg.
- Dapagliflozin 10 mg/metformin hydrochloride 1000 mg.
Store Xigduo XR at room temperature away from excess heat and moisture. Keep the tablets in the original container and out of reach of children and pets.
Xigduo XR is manufactured by AstraZeneca.
There is currently no approved generic version of Xigduo XR available in the United States.
Do not stop taking Xigduo XR without speaking to your healthcare provider. Stopping suddenly may cause your blood glucose to rise. Your provider may recommend another antidiabetic medication or adjust your treatment plan.
Talk to your doctor if you are already pregnant or if you have any plans to become pregnant. They will know or recommend what is best for you. Xigduo XR is generally not recommended during the second and third trimesters because dapagliflozin may affect kidney development in an unborn baby.
Talk to your doctor if you are already breastfeeding or if you have any plans to breastfeed. They will know or recommend what is best for you. Because dapagliflozin may pass into breast milk and could affect the infant’s kidneys, breastfeeding is usually not recommended while taking this medication.
