What is Zestoretic?
Zestoretic (lisinopril/hydrochlorothiazide) is a prescription medication used to treat high blood pressure (hypertension). Managing blood pressure is important to reduce the risk of serious complications such as heart attack, heart failure, and kidney disease.
Zestoretic may be used when monotherapy is not enough to control blood pressure. It is part of a broader treatment plan that includes lifestyle changes such as diet, exercise, and weight management.

Important information about Zestoretic
The active ingredients in Zestoretic are lisinopril and hydrochlorothiazide.
Lisinopril is an angiotensin-converting enzyme (ACE) inhibitor. Angiotensin II is a substance that narrows blood vessels. By lowering angiotensin II levels, lisinopril helps relax and widen blood vessels to improve blood flow.
Hydrochlorothiazide (HCTZ) is a thiazide diuretic, also known as a water pill. It helps the kidneys remove excess salt and water from the body through urine. This reduces fluid buildup, lowers blood volume, and helps decrease blood pressure as a result.
Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Zestoretic. However, the following is a synopsis of the general guidelines for the use of Zestoretic:
- Zestoretic is usually taken once daily, with or without food. Try to take it at the same time each day.
- Because hydrochlorothiazide is a diuretic, you may notice increased urination, especially when starting treatment.
- If you miss a dose, take it as soon as you remember. If it is close to your next dose, skip the missed dose and take the next dose at your regular time.
- Continue taking the medication even if you feel well. High blood pressure often has no symptoms.
- Patients on dialysis or with significant renal impairment should be closely monitored.
Some mild or common side effects of Zestoretic seen in clinical trials include:
- Dry cough.
- Lightheadedness.
- Headache.
- Drowsiness.
- Muscle cramps.
- Dry mouth.
- Constipation.
- Increased urination.
- Electrolyte changes such as hypokalemia, hyponatremia, or hypomagnesemia.
Some individuals may experience serious adverse reactions while using Zestoretic. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:
- Angioedema - swelling of the face, lips, throat, or intestinal angioedema causing abdominal pain.
- Severe low blood pressure (hypotension), causing fainting.
- High potassium levels (hyperkalemia).
- Hypercalcemia (high calcium in the blood).
- Kidney problems or worsening renal function.
- Jaundice or signs of liver problems.
- Eye problems - acute myopia, angle-closure glaucoma with eye pain or blurred vision.
- Severe allergic reaction - skin rash, trouble breathing.
- Irregular heart rhythms or arrhythmias.
- Increased risk of getting skin cancer.
- Low levels of a type of white blood cell (called neutrophils).
These are not all of the possible side effects of Zestoretic. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.
Before starting Zestoretic, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:
- Kidney disease or renal impairment.
- Liver disease.
- Heart failure.
- Electrolyte imbalances.
- History of angioedema.
- Diabetes or issues with blood glucose.
- Collagen vascular disease or systemic lupus erythematosus.
- Glaucoma or vision problems.
- Hypertrophic cardiomyopathy or aortic stenosis.
Boxed Warning
- Zestoretic carries an FDA boxed warning regarding use during pregnancy. ACE inhibitors can cause injury or death to an unborn baby when used during the second and third trimesters. The medication should be stopped as soon as pregnancy is detected.
- History of angioedema related to ACE inhibitor use.
- Anuria (inability to produce urine).
- Hypersensitivity to lisinopril, hydrochlorothiazide, or other sulfonamide-derived drugs.
- Use with aliskiren in patients with diabetes.
- Have taken a medicine that contains sacubitril (such as Entresto) within the past 36 hours.
Zestoretic begins to lower blood pressure within a few hours after the first dose. However, it may take several weeks to see the full antihypertensive effect. Regular monitoring by a healthcare professional is important to ensure the medication is working properly.
When Zestoretic is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:
- Aliskiren.
- Sacubitril.
- Non-steroidal anti-inflammatory drugs (NSAIDs) (ibuprofen, indomethacin)
- Potassium supplements or salt substitutes.
- Diuretics.
- Other antihypertensive medications.
- Lithium.
- Angiotensin receptor blockers (ARB) (valsartan).
- Cholesterol medications (cholestyramine, colestipol).
- mTOR inhibitors (sirolimus, everolimus, temsirolimus).
- Corticosteroids.
Zestoretic tablets are available in different strengths, including combinations of lisinopril and hydrochlorothiazide, such as:
- 10 mg/12.5 mg.
- 20 mg/12.5 mg.
- 20 mg/25 mg.
Store Zestoretic at room temperature away from moisture and heat. Keep the medication in its original container and out of reach of children.
Zestoretic is a brand-name medication originally developed by Atnahs Pharma.
Generic versions of lisinopril/hydrochlorothiazide are available.
Do not stop taking Zestoretic without speaking with your healthcare provider. Stopping suddenly may lead to increased blood pressure. Your healthcare provider may recommend gradual discontinuation or switch you to another antihypertensive medication if needed.
ACE inhibitors can cause serious harm to an unborn baby, especially during the second and third trimesters. If you find out you are pregnant while taking Zestoretic, stop the medication and let your provider know immediately.
Small amounts of medication may pass into breast milk. Talk to your doctor if you are already breastfeeding or if you have any plans to breastfeed. They will know or recommend what is best for you.
