Important information about Zanidip
Zanidip contains lercanidipine hydrochloride, which belongs to a group of medicines called calcium channel blockers (CCBs) of the dihydropyridine type. It works by relaxing blood vessels, allowing blood to flow more easily, and helping to lower high blood pressure.
Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Zanidip. However, the following is a synopsis of the general guidelines for the use of Zanidip:
- Take Zanidip once daily, preferably at the same time each day.
- Take it at least 15 minutes before meals, as a high-fat meal can increase how much of the drug is absorbed.
- Swallow the film-coated tablet whole with water.
- Avoid grapefruit juice while taking Zanidip, as it can increase the effects of the medication.
- If you miss a dose, take it as soon as you remember. If it is close to your next dose, skip the missed dose and take the next dose at the regular time. Do not take two doses at once.
Some mild or common side effects of Zanidip include:
- Headache.
- Flushing.
- Heart palpitations.
- Dizziness.
- Sleepiness.
- Swelling of the ankles (edema).
Some individuals may experience serious side effects while using Zanidip. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:
- Signs of a heart attack include chest pain, shortness of breath, and pain radiating to your shoulder, neck, or jaw.
- Allergic reactions, such as hives or swelling of the face.
These are not all of the possible side effects of Zanidip. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.
Before starting Zanidip, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including:
- Heart disease, including angina pectoris, left ventricular dysfunction, heart failure, or arrhythmias.
- Peritoneal dialysis.
- Intolerance to lactose monohydrate, microcrystalline cellulose, sodium starch glycolate, magnesium stearate, povidone, hypromellose, macrogol, talc, or titanium dioxide.
- Liver or kidney disease.
- Taking other medications that affect heart rate or blood pressure, such as beta-blockers or diuretics.
- Unstable angina.
- Severe heart failure.

- Taking strong CYP3A4 inhibitors, ciclosporin, or grapefruit juice.
Zanidip starts lowering blood pressure within a few hours, but the full effects may take several days to weeks.
When Zanidip is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:
- Strong CYP3A4 inhibitors such as ketoconazole, itraconazole, erythromycin, clarithromycin, ritonavir, and troleandomycin can increase levels and side effects of Zanidip.
- Co-administration with ciclosporin may increase levels of both drugs.
- Strong CYP3A4 inducers such as rifampicin, carbamazepine, and phenytoin may reduce the effectiveness of Zanidip.
- CYP3A4 substrates such as astemizole and terfenadine should be used with caution with Zanidip.
- Midazolam may have increased effects when used with Zanidip.
- Simvastatin may have higher levels and side effects when used with Zanidip, and it should be taken at a different time of day if used together.
- High doses of cimetidine may increase Zanidip levels.
- Zanidip may increase digoxin levels.
- Beta-blockers such as metoprolol and alcohol may enhance the blood pressure-lowering effects of Zanidip.
- Antiarrhythmics such as amiodarone, quinidine, and sotalol may affect heart rhythm when used together.
Zanidip is available as film-coated tablets in strengths such as 10 mg and 20 mg of lercanidipine hydrochloride.

Barring any of the above serious side effects or under the advice of your physician, do not stop taking Zanidip suddenly.
While no studies have been done for the use of Zanidip in pregnancy, other calcium channel blockers are known to cause irreversible fetal malformations, so it should not be used in pregnancy. Females who are able to get pregnant should use effective contraception while on this medication.
