Yupelri

The active ingredient in Yupelri is revefenacin. Revefenacin belongs to a class of medications known as long-acting muscarinic antagonists (LAMA). The muscles used for breathing have receptors on their surfaces called muscarinic receptors. When activated, these receptors signal to the airways to narrow and constrict, which can be especially problematic in those with breathing problems like COPD. When these receptors are blocked, the airways will relax and widen, making it easier to breathe.

It should be noted that LAMAs work in the long term. They cannot work quickly enough to relieve a sudden onset of breathing problems like wheezing, cough, or shortness of breath. You will still need to keep a rescue inhaler on hand in case of situations like these.

However, the following is a synopsis of the general guidelines for the use of Yupelri:

• Yupelri should be used exactly as prescribed.
• A typical dose for most patients is one vial that is inhaled once daily using a jet nebulizer with a mouthpiece. The nebulizer converts the inhalation solution into a mist that you can then breathe in. Ensure the solution is clear or colourless. If it is not, it may be expired or damaged medication. Do not swallow or inject the solution or mix it with other medications. Your pharmacist or doctor will review with you how to use this device.
• Do not open the vials until you are ready to use them. Keep the vials in their foil packaging to preserve product integrity and freshness.
• If you miss a dose, take it as soon as you remember. However, if it is almost time for your next dose, skip the missed dose and continue with your regular dosing schedule. Do not take a double dose to make up for a missed one.

The most common side effects of Yupelri include:

• Cough
• Cold symptoms such as a runny nose, sneezing, or sore throat
• Upper respiratory tract infection
• Headache
• Back pain

Some individuals may experience serious side effects while using Yupelri. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:

• Sudden breathing problems after using Yupelri (paradoxical bronchospasm). If this occurs, use a short-acting bronchodilator like albuterol immediately and discontinue Yulpelri.
• Decreased effectiveness of your short-acting COPD medications or greater use of them. These could be signs of worsening lung function.
• Signs of angle narrow-angle glaucoma such as eye pain or discomfort, red eyes, blurred vision, seeing halos, and/or nausea or vomiting.
• Urinary problems such as difficulty urinating, urinary retention, urinating more than usual, or painful urination.
• Signs of a serious allergic reaction such as hives, rash, difficulty breathing, or swelling of the face, lips, eyes, tongue, throat, or other parts of the body.

• Eye problems, especially glaucoma
• Problems with your prostate or bladder
• Liver problems
• Are pregnant or planning to become pregnant
• Are breastfeeding or planning to breastfeed

Do not use Yupelri if you have an allergy to revefenacin or any of the other ingredients in Yupelri.

Yupelri will start to work right after the first dose. However, as it is a long-term treatment, it may take a longer period of use before you see the full benefits of Yupelri.

When Yupelri is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:

• Anticholinergics such as atropine, tiotropium, ipratropium, aclidinium, umeclidinium, and glycopyrrolate
• Rifampin
• Cyclosporine

Yupelri is available as a solution for inhalation that contains 175 mcg of revefenacin for every 3 mL of fluid.

Yupelri should be stored at room temperature, away from light and moisture, and out of reach of children. It should be kept in its original foil packaging until you are ready to use it. Any vials that contain fluid that is no longer clear or colorless should be discarded. Contact your pharmacy for proper disposal.

Yupelri is manufactured by the pharmaceutical company Mylan Specialty, a Vitaris Company.

Currently, there is no generic available for the brand-name Yupelri.
