What is Loestrin?

What is Loestrin?

Loestrin (ethinyl estradiol/norethindrone acetate) is a prescription birth control pill used to prevent pregnancy. It is a combination oral contraceptive that contains both an estrogen and a progestin. Loestrin helps regulate the menstrual cycle and is also sometimes used to treat painful menstrual periods (dysmenorrhea) or other hormone-related concerns.

Some products, such as Lo Loestrin Fe, include ferrous fumarate tablets in the pill pack. These iron tablets are placebo pills and do not contain active hormones. They are included to help keep you in the habit of taking a pill every day.

Loestrin does not protect against sexually transmitted diseases. If protection from infections is needed, a non-hormonal method such as condoms should be used.

Important information about Loestrin

Loestrin contains two hormones: ethinyl estradiol, an estrogen, and norethindrone acetate, a progestin. Together, these hormones prevent pregnancy in several ways.

First, they stop ovulation. Ovulation is when the ovary releases an egg each month. Without ovulation, there is no egg for sperm to fertilize.

Second, Loestrin thickens the cervical mucus. This makes it harder for sperm to reach an egg.

Third, it changes the lining of the uterus, which reduces the chance that a fertilized egg can attach and grow.

Follow all directions given to you by your doctor and read all documents provided, including the full Prescribing Information, FDA-Approved Product Labeling, Instructions for Use, and Medication Guide before using Loestrin. However, the following is a synopsis of the general guidelines for the use of Loestrin:

  • Loestrin is taken by mouth once daily, at about the same time each day. Each pill pack contains a set number of active tablets and, in some products, placebo or ferrous fumarate tablets.
  • You may start taking Loestrin on the first day of your menstrual period (first day start) or on the first Sunday after your period begins. Your healthcare provider will tell you which contraceptive method to start with is best for you. If you do not start on the first day of your cycle, you may need to use a backup method, such as a non-hormonal birth control method, for the first 7 days.
  • Take one tablet daily in the order shown in the pill pack. When you finish a pack, start a new pack the next day, even if you are still bleeding.
  • If you miss pills, follow the instructions in the prescribing information. Missing pills increases the risk of pregnancy. You may need a backup method if you miss pills.

Some mild or common side effects of Loestrin seen in clinical trials as compared to placebo include:

  • Nausea.
  • Vomiting.
  • Breast tenderness.
  • Headache.
  • Breakthrough bleeding.
  • Changes in menstrual periods.
  • Amenorrhea (missed periods).
  • Weight gain.
  • Mood changes.

Some individuals may experience serious side effects while using Loestrin. These side effects may require immediate medical attention. If you experience any of the following, contact your healthcare provider or seek emergency medical help:

  • Signs of blood clots - leg pain, swelling, chest pain, shortness of breath.
  • Sudden severe headache or vomiting, dizziness or fainting, disturbances of vision or speech, weakness, or numbness in an arm or leg, which are signs of a stroke.
  • Signs of a heart attack.
  • Yellowing of the skin or eyes (jaundice).
  • Symptoms of high blood pressure.
  • Allergic reaction - rash, hives, or trouble breathing.

These are not all of the possible side effects of Loestrin. You should always seek medical advice from your healthcare provider for any questions or concerns about your medical condition or treatment. Read all patient information, medication guides, or drug information sheets that come with this medication. You can also report adverse effects to the Food and Drug Administration at www.fda.gov/medwatch or 1-800-FDA-1088.

Before starting Loestrin, it is crucial to discuss any existing health conditions with your healthcare provider. You should be sure that your healthcare provider is aware of all your medical conditions, including if you have:

  • History of blood clots or blood vessel problems.
  • High blood pressure (hypertension).
  • Heart disease or previous heart attack.
  • Breast cancer or other hormone-sensitive cancers.
  • Liver disease, liver tumors, or hepatitis C.
  • Diabetes or blood sugar problems.
  • Migraine with aura.
  • Smoking history, especially if over age 35.
  • Recent pregnancy or if you are breastfeeding.

Women taking certain hepatitis C treatments containing ombitasvir, dasabuvir, and ritonavir should not use combination hormonal birth control due to the risk of liver problems.

Boxed Warning

  • Cigarette smoking increases the risk of serious cardiovascular events from combination oral contraceptive use. This risk increases with age and the number of cigarettes smoked, and is especially significant in women over 35 years old. Women who use Loestrin should not smoke.

Loestrin should not be used in women who:

  • History of blood clots, stroke, or heart attack
  • Certain heart or blood vessel disorders
  • Known or suspected breast cancer
  • Liver disease or liver tumors
  • Unexplained vaginal bleeding
  • Pregnant
  • Taking certain hepatitis C drug combinations containing ombitasvir, dasabuvir, and ritonavir

Your healthcare provider will review your medical history to determine if Loestrin is safe for you.

If you start Loestrin on the first day of your menstrual cycle, it works right away. If you start at another time, you may need a backup method for the first 7 days.

When Loestrin is taken with other prescription drugs, over-the-counter medications, vitamins, and supplements, it may change how they work or increase the frequency or severity of side effects. Make sure that you tell your healthcare professional about anything that you are taking to avoid any negative drug interactions, including:

  • Rifampin.
  • Carbamazepine.
  • Phenytoin.
  • Barbiturates.
  • Griseofulvin.
  • Ritonavir.
  • St. John’s wort.
  • Some supplements.

These drugs may reduce the effectiveness of hormonal birth control and increase the risk of pregnancy. You may need a backup method while taking these medications.

Loestrin is available in several strengths, including lower-dose options such as Lo Loestrin Fe. The exact amount of ethinyl estradiol and norethindrone acetate varies by product. Some packs include ferrous fumarate tablets as placebo tablets during the last days of the cycle. Your healthcare provider will choose the dose that is right for you.

Store Loestrin at room temperature, away from heat and moisture. Keep the pill pack in its original packaging and out of reach of children.

Loestrin is manufactured by Teva Pharmaceuticals.

Generic versions containing norethindrone acetate and ethinyl estradiol are available.

If you wish to stop taking Loestrin, talk with your healthcare provider first. You can stop at any time, but you may become pregnant soon after stopping. Consider another birth control method if you do not want to become pregnant.

Loestrin should not be used during pregnancy. Let your doctor know if you are already pregnant or if you have any plans to become pregnant.

Combination oral contraceptives may reduce the amount of breast milk. Small amounts of hormones may pass into breast milk. Talk to your doctor if you are already breastfeeding or if you have any plans to breastfeed. They will know or recommend what is best for you. In some cases, a progestin-only pill or non-hormonal method such as an IUD may be recommended.
